The town stretches along the banks of the Mahajilo River, a major tributary of the Tsiribihina, and serves as a strategic gateway for river expeditions into the interior. Economically, the settlement is strongly characterized by agriculture, with the cultivation of tobacco, rice, and zebu cattle farming being the primary sources of income for the population. Geographically, Miandrivazo lies in a wide basin, leading to extreme temperatures that frequently exceed the 40-degree Celsius mark. Historically, the name is closely tied to the Merina King Radama I, who reportedly waited here for his future wife, giving the place its name (loosely translated as 'waiting for a wife'). For tourism, the city is of great importance as the starting point for multi-day pirogue trips down the Tsiribihina River, which pass through spectacular gorges and remote villages. Despite its remoteness, Miandrivazo is connected via National Road 34 to the central highlands and the city of Antsirabe.
